Arizona Long Term Care System

LOCATED IN PHOENIX. SERVES YAVAPAI COUNTY. a resource of Arizona Health Care Cost Containment System (AHCCCS).

The Arizona Long Term Care System (ALTCS, pronounced ALL-Tecs) is health insurance for individuals who are age 65 or older, or who have a disability, and who require nursing facility level of care. Services may be provided in an institution or in a home or community-based setting.

Provide ongoing care management and support, communicate with doctors/healthcare providers, arrange and monitor in-home care.

8:00 AM - 4:00 PM (5 days a week). On call 24 hours for clients.

Case Management

Located in Prescott. Serves yavapai county. A resource of NACOG (Area Agency on Aging).

Proveds case management. Community members need to call the Intake and Referral line for information and to start the referral process for services. The majority of our services are for those 60 and older.

DEMENTIA SCREENING CLINIC AND DEMENTIA SCREENING CLINIC

LOCATED IN PRESCOTT. SERVES QUAD CITIES. A RESOURCE OF DIGNITY HEALTH YRMC.

YRMC features geriatrics programs that include: Alzheimer’s and Dementia Care: provides the support system people with dementia and their care partners need to continue living in the community. The DSC team includes a neuropsychologist, palliative medicine physician, pharmacist, social worker and nurse navigator.
